$500/year or $50/month (Scholarships available)

To be considered for the SWANN Supporter/Provider track, you must submit a formal application including credentials, practice philosophy, and how your work aligns with SWANN’s mission.

If your application is approved, you'll be offered 3 free months of the Supporter Membership.
After the first 3 months, you can choose to stay at the Supporter tier or you can apply to become a SWANN Provider.

Anytime after your first 3 months, you can apply to become a SWANN Provider. Applicants must:
- Fill out an expanded application
- Attend 3+ SWANN meetings or events
- Meet 1-on-1 with the 4 Founders
- Engage in at least 1 collaborative patient case review or provider discussion
Completed applications will be reviewed by the Membership Committee once a quarter.
